Under the direction of the Director of Care Management, the Community Health Worker (CHW) helps patients access and navigate community services. The CHW conducts outreach, provides information and support to community members, and assists in data collection to identify health needs and promote process improvement, with a particular focus on high-risk populations. Essential Duties and Responsibilities include establishing and maintaining relationships with community partners, patients and their families while providing general healthy behavior support; assisting families, groups, and communities in accessing both social and health related services; assessing client’s needs and referring clients to community resources and other agencies to meet identified needs, developing processes for assessing Social Determinants of Health in all settings; attending community meetings and appropriate events (ex: health fairs, etc.) to understand community issues and provide information to community regarding possible resources; advocating for individual and community health needs; acting as a liaison between Phelps Health, other community-based organizations and the community they serve; developing referral process and follow-up for identified patient needs; distributing flyers, brochures, and other informational pieces appropriate for the target population in the community served by Phelps Health; and performing other related duties as assigned to support goals. Education required is an Associate degree in public health, social work, psychology, sociology, or a related field. Experience with social services or public health systems preferred. Certification/License requirements include Basic Life Support (BLS) must be obtained within first 90 days of employment (BLS training is provided) and Certification as Community Health Worker must be obtained within first 6 months of employment. Mental/Physical Requirements include considerable mental concentration for sustained periods of time with frequent interruptions, light lifting (15 lbs.), standing, sitting, walking, reading, and speaking. Working Conditions include typical office conditions with noise and distractions, possible eye strain or other discomfort from constant use of computer screens, attending meetings in public, and possible exposure to infectious or communicable disease in the course of meeting with patients in any setting across the organization.
