Coordinates health care system outreach and education for the community. Administers programs to develop community relationships with healthcare providers. Drives health service awareness to assist members in maintaining optimal health outcomes. Executes both routine and non-routine business support tasks for the Community Health area under limited supervision, referring deviations from standard practices to managers. Follows area protocols, standards, and policies to provide effective and timely support. Builds and maintains effective working relationships with members, providers, case managers, and agency representatives, and assists with care plan implementation. Manages initiatives to reduce culture and socio-economic barriers between members and providers and identifies gaps and opportunities to strengthen systems of care with the community. Maintains member health records, compiles reports, develops educational materials, and completes program documentation. Takes direction to execute techniques, processes, and responsibilities. Must reside in the following counties- Graves, McCracken, Lyon, Muhlenberg, Todd, Warren, Butler, Ohio, Daviess. Minimum Requirements: Working knowledge of problem solving and decision making skills. Education: High school diploma or equivalent required. Required Qualifications: Valid KY CHW certification. Preferred Qualifications: 1+ years experience as CHW.
