Job Summary

Make a meaningful impact in your community—join Acentra Health as a Clinical Evaluator (PASRR) in Kitsap Lake, Washington. In this hybrid role, you will conduct in-person assessments in nursing homes, hospitals, and community-based settings throughout Kitsap Lake and its surrounding areas.

As a Clinical Evaluator, you will conduct face-to-face PASRR Level II assessments to determine appropriate placements and next levels of care for individuals with complex needs. Your primary focus will be completing clinical evaluations for individuals across nursing homes, hospitals, and community-based settings. These evaluations are crucial in determining the next level of care, improving patient well-being, and supporting better health outcomes.

 

This PRN hybrid role offers flexibility and purpose, combining remote work from your home office with local travel as needed. All travel expenses—including mileage and parking are covered and reimbursed by Acentra Health. In addition, Acentra provides all necessary technology and equipment for your home office, including a laptop for use in the field. 

 

Job Responsibilities

• As a Clinical Evaluator (PASRR), you will conduct accurate screenings and evaluations, developing informed level-of-care recommendations based on your clinical assessments.

• Complete precise PASRR reviews by assessing and determining the appropriate level of care based on patients’ physical, behavioral, and psychological functioning. 

• Review medical records, and interact with healthcare professionals, patients, their families, and other support systems to conduct accurate screenings, evaluations and determinations Assess and analyze psychological, emotional, behavioral and physical functioning.
• Ensure compliance with contractual and regulatory standards while developing level of care recommendations.
• Assess and analyze patients' psychological, emotional, behavioral, and physical functioning to inform clinical decisions.
• Provide well-informed level of care recommendations based on clinical evaluations.
• Ensuring compliance with established workflows, including re-evaluations of records based on psychiatric input.
• Collaborate effectively with healthcare professionals, patients, families, and internal stakeholders as needed.
• Efficiently utilize computer equipment and web-based software applications to perform job duties
• Leverage technology by utilizing the state's electronic system and our proprietary platforms to efficiently manage and track all assessment activities. .
• Efficiently prioritize and execute tasks with attention to detail.
• The list of accountabilities is not intended to be all-inclusive and may be expanded to include other duties that management may deem necessary.

Qualifications:

 

Required Qualifications, Knowledge, and Experience

• Requires an active, unrestricted license in the State of Washington in one of the following fields: Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), Social Worker Independent Clinical License (SWICL), Licensed Marriage and Family Therapist (LMFT), Licensed Independent Clinical Social Worker (LICSW), Licensed Clinical Psychologist (LCP), or Licensed Mental Health Professional (LMHP).
• Graduation from an accredited college with a relevant degree program and a minimum of 2+ years of experience in assessing individuals with mental health or psychiatric disorders, including complex differential diagnoses related to substance use, medical conditions, cognitive impairments, and dementia.
• Minimum 1 year of experience working with individuals with intellectual or developmental disabilities or related conditions.
• Solid understanding of standards of practice related to Level of Care determinations and familiarity with standards pertaining to mental health, geriatrics, and other conditions that often require long-term care placement.
• Demonstrated expertise in assessing and analyzing an individual’s psychological, emotional, behavioral, and physical functioning.
• Working knowledge and understanding of diagnosing psychiatric disorders in accordance with the most current edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM) applicable in the state. .
• Excellent communication skills, with the ability to convey complex information clearly and effectively to individuals and groups through verbal, written, and electronic formats.
• Proficiency in medical record abstraction, with solid knowledge of medical record organization, terminology, and common disease processes.
• Experience working with geriatric populations is preferred, particularly in long-term care, behavioral health, or community-based service settings.
• Experience with public mental health and medical programs, including familiarity with public sector services and community resources, is preferred.
• Skilled in using Microsoft Office Suite including Excel, Word, PowerPoint, Outlook and other web-based software and platforms.

Compensation

The compensation for this role is $41.00 to $44.00 per hour

Based on our compensation program, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
